With The Practice Passport, Novum provides wellness professionals the opportunity to use professional space on a short-term basis. Novum is committed to providing a welcoming, professional, and flexible space where independent professionals can build and grow their practices. Every provider who uses the space is responsible for their own clients, services, and business. Novum provides the space only and does not supervise, employ, or take responsibility for the services any practitioners provide. We ask for your license number, proof of professional liability insurance, and your specialties so we can confirm that everyone using the space is properly licensed and insured, keep the space safe and professional for all providers and their clients, and understand the needs of the practitioners we serve.
What you're agreeing to
By submitting this interest form, you confirm that the information you've provided is accurate and that you hold a current license in good standing. You'll need active professional liability insurance before your first use of the space, and you agree to keep it in place and notify Novum promptly if your license or coverage changes or lapses. If you don't currently have liability insurance, you need to obtain coverage before Novum will permit you to use the space. You are responsible for your own conduct, your clients, and any damage you cause to the space. Your use of Novum is governed by the Practice Passport Professional Space Use License, which we encourage you to read in full before signing. Novum LLC will use your information only to manage your use of the space and will not share or sell your information with outside parties.
